***How to Get Diapers***

If you are a SOCIAL WORKER, PLEASE HAVE YOUR CLIENT CONTACT US DIRECTLY!

 

OUR NEW FAX NUMBER is 866-575-2230

How it works: Individuals who want free diapers will have to sign up. It’s easy. You can sign up even if your child does not attend one of the centers listed below.

Here’s what you need:

• Our Application (DOWNLOAD THE CLIENT APPLICATION BELOW)

• Your ID- a driver’s license or State-issued ID
• Your child’s birth certificate (or birth announcement if newborn)
• Proof of income. Either copy of paystub or Unemployment or form stating you receive TANF or Food Stamps

FAX THIS INFO TO: 866-575-2230

Or Mail the application, copy of ID, copy of child’s birth certificate and copy of proof of income to:

Atlanta Diaper Relief, 8491 Hospital Drive, Suite 241, Douglasville, GA 30134

You would also have to show proof of income and residency that you live in Douglas, Cobb, Fulton or Gwinnett county.

Agencies - If you are an agency looking to become a member of the Diaper Distribution Network (DDN), please download  an application and  email director@atlantadiaperrelief.org or mail it to us at Atlanta Diaper Relief, 8491 Hospital Drive, Suite 241, Douglasville, GA 30134.

Once you have been approved to receive diapers, someone will call you and you will let us know WHERE you would like to pick up your diapers. The locations are listed BELOW.
